Mazzios Survey Rules

If you want to complete the survey successfully, the rules are very important. Although the survey is straightforward, there are some rules for participants. These are not quite strict. Just follow them to get your Mazzio coupon.

You should have a recent purchase receipt of Mazzio’s.
You should have a valid receipt with Survey ID. There is no other way to enter the Customer loyalty index survey without it.
One person can use one receipt only once.
The customer must be a legal resident of the USA.
The prize coupon is non-transferable.

Mazzios Survey Requirements

Since it’s an online survey you need an internet connection.
The second thing that you would know by now is the purchase receipt with Survey ID.
You can run it either on a computer or your smartphone.
The survey is available only in English, so you should know that to answer the questionnaire.
You will also need a valid email address to receive the reward coupon after completion of the survey.

How to participate in TellMazzios Survey

Now we know all the requirements and rules of the survey. Now you are ready for the survey. Here are the steps you will follow.

Step 1: Enter the 5-digit coupon code from the purchase receipt and click on start.

Step 2: Now enter the date and time of the visit from that receipt. Also, select the type of your visit, and proceed to the next page.

Step 3: In the next few pages, you will answer questions about your purchase. Like if you tried the buffet or if you made any special request.

Step 4: After you are done with those questions, there are a bunch of questions, where you rate your satisfaction.

Step 5: If you face any problem, you can point that out too. And later describe it in the comment box.

Step 6: At the end, there is a box where you write about the reason for your ratings. This box is optional, and you can leave it blank if you like.

Step 7: Lastly, there may be a few classification questions.

Step 8: Now enter and confirm your email address where you will receive the validation code.

Step 9: Click on the submit button. On successful submission, you will get a message at the end.

Ken Selby was a school teacher when he decided he wanted to open a business at his birthplace, Tulsa, Oklahoma. Initially, he worked online for the new business and taught full-time. Mazzio’s pizza became a reality from a concept in 1979. And this new change became popular quickly. In the coming decade, the business grew and expanded to 150 more locations. His new and thicker pizzas with extra toppings were enjoyed by many. Mazzio’s also introduced the made-to-order pasta. Their salads and Calzone rings were some other popular additions to the menu. Currently, their menu has many different and innovative products. For desserts, they have Brookie, which is a 9-inch brownie cookie. Another one is Cinnaboli, a cinnamon and sugar dessert. Customer Care

Customer support is a big part of any restaurant experience. Mazzio provides multiple ways for its customers to contact them. First is their comment card. There you have to give information about your purchase. Like the location, visiting time, and the date. Based on the information, you will select the restaurant address. After this, there is a form where you have to give your name, email address, and other contact details if necessary. Proceed to the next page where there is a comment box. Describe your issue in that box. If you need to, you can attach three photos. Lastly, you rate your experience out of five and submit.
